Licensed Practical Nurse (LPN)

At Auburn Crest Primary & Palliative Care, we bring medical care directly to seniors in the assisted living and memory care communities they call home. We're looking for a compassionate Licensed Practical Nurse to be the steady, trusted connection between our patients, their families, community staff, and our Nurse Practitioners across Ada & Canyon County.

What You'll Do

• Serve as the go-to clinical contact for the assisted living and memory care communities in your county
• Partner with our RNs and Nurse Practitioners to schedule and prioritize patient visits
• Process referrals, medication requests, and refills as directed by providers
• Coordinate labs, durable medical equipment (DME), and other clinical resources
• Send, track, and follow up on provider orders
• Educate patients, families, and caregivers so everyone feels confident in the care plan
• Keep accurate, timely documentation in our electronic medical record

What You Bring

• Active LPN license in Idaho
• 1+ year of nursing experience preferred
• Experience in geriatrics, assisted living, memory care, palliative care, or hospice is a plus
• Current CPR/BLS certification
• Strong organization and clear communication, both verbal and written
• Valid driver's license, auto insurance, and reliable transportation for local travel
