Assessment Clinician Triage, Peace Hospital, PRN

Job Description The Assessment Clinician I evaluates individuals seeking behavioral health/substance use treatment, seeks justification for admission, and coordinates level of care expectations with physicians, Assessment Referral Center, personnel, patient care units, and external agencies. The clinician also develops relationships with community partners by consistently providing quality services. The Assessment Clinician I conducts insurance pre‑certifications for individuals evaluated and may provide assessment services for various patient populations including pediatric, adolescent, adult, and geriatric. UofL Health Medical Staff and Board of Directors under the EMTALA policy consider Assessment Referral Center Clinicians elected to be “Qualified Medical Persons” to conduct emergency behavioral health and substance use screening exams. Essential Functions Evaluate individuals seeking behavioral health/substance use treatment and seek justification for admission. Conduct evaluations at local hospitals, UofL Peace Hospital and other community agencies and facilities. Refer patients to appropriate services and assist in the intake process for admitted patients. Determine provisional diagnosis and discuss treatment recommendations with patient, family, and medical providers. Coordinate level of care expectations with physicians, Assessment Referral Center, personnel, patient care units, and external agencies. Develop relationships with community partners by consistently providing quality services. Assess the patient’s psychological, social, and treatment needs through analysis of data collected from patients, families, and collateral sources such as school, outpatient therapists, and community providers. Establish provisional diagnosis based upon thorough level of care assessment. Record and communicate clinical information to physician and interdisciplinary treatment team as indicated. Perform insurance prior authorizations for patients. Present detailed clinical information to medical providers and collaborate with nursing staff to assess the most appropriate level of care and medical needs of the patient. Other Functions Maintain professional development and education. Demonstrate proficiency in determining the most appropriate level of care and provide referrals based on clinical assessments. Communicate relevant information to patient and family regarding treatment programs and patient rights issues. Demonstrate effective understanding of legal issues associated with assessment and/or admission for voluntary status, EMTALA, and Mental Inquest Warrant processes. Provide assessment services for various patient populations including pediatric, adolescent, adult, and geriatric as needed. Maintain compliance with all company policies, procedures and standards of conduct. Comply with HIPAA privacy and security requirements to maintain confidentiality at all times. Perform other duties as assigned. Education and Experience Master’s Degree in Social Work, Counseling Psychology, Clinical Psychology, Family and Marriage Therapy, Mental Health Counseling, or other related fields required. One year of experience in the mental health field and previous experience in crisis intervention or an assessment/diagnostic setting (preferred). Licensure & Certification Valid Driver’s License required for Mobile Assessment Clinicians only. Basic Life Support (BLS) accredited by the American Heart Association (AHA) required. Facility‑provided Safe Crisis Management training required. Job Competency Psychology – knowledge of human behavior, individual differences, learning and motivation, research methods, and assessment of behavioral and affective disorders. Therapy and Counseling – knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions. Social Perceptiveness – awareness of others’ reactions and understanding why they react as they do. Active Listening – giving full attention to what others are saying, asking questions, and not interrupting inappropriately. Critical Thinking – use of logic and reasoning to identify strengths and weaknesses of alternative solutions. Service Orientation – actively looking for ways to help people. Problem Sensitivity – recognizing when something is wrong or likely to go wrong. Language Ability – effective communication in both verbal and written formats. Reasoning Ability – breaking down problems or tasks; scanning own knowledge and experience to identify causes and consequences. Computer Skills – strong working knowledge of Microsoft Office products (Word, Excel, PowerPoint) and ability to learn additional systems including electronic medical record systems and telehealth programs. Additional Responsibilities Demonstrate a commitment to service, organization values and professionalism through appropriate conduct and demeanor at all times. Maintain confidentiality and protect sensitive data at all times. Adhere to organizational and department‑specific safety standards and guidelines. Work collaboratively and support the efforts of team members.
