RN - Hospice

This position involves providing compassionate care to patients in a hospice setting, focusing on pain symptom management and holistic support for individuals with terminal conditions. Responsibilities Pain Symptom Management: Assess patients and administer medications/treatments to ensure comfort and alleviate suffering. Care Planning: Develop and update customized patient care plans in collaboration with physicians, social workers, and other team members. Family Support: Educate family caregivers on disease progression, what to expect, and medication management. Crisis Intervention: Respond quickly to sudden changes in a patient's condition to manage pain and provide crisis support. Documentation: Keep accurate, up-to-date patient medical records and facilitate communication between the care team. Emotional Spiritual Care: Offer compassionate companionship, grief counseling, and post-mortem support to grieving relatives. Schedule This position requires a Day shift, working 8 hours a day from 08:30 AM to 05:00 PM, Monday through Friday. On-call duty is expected for one full week every month, which includes evenings and weekends.
