Our work hours typically span Monday through Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evenings or weekends depending on client requirements. On-call availability may be needed to respond to urgent situations or support clients during off-hours, emphasizing the commitment to flexible and responsive service.

Key Responsibilities\n

As an LCSW at Heartfelt Connections Therapy, you will engage in a variety of essential tasks, including:

• Working collaboratively with seniors and their families to create tailored care plans that reflect their personal goals and preferences.

• Providing individual and family counseling to assist clients in navigating life transitions, managing challenges, and strengthening resilience.

• Advocating for clients’ rights, helping them access critical community resources, and ensuring they receive necessary services.

• Engaging with an interdisciplinary team that includes nurses, therapists, and healthcare providers to coordinate comprehensive care plans effectively.

• Conducting regular follow-up visits to monitor clients’ conditions, evaluate care effectiveness, and address any safety concerns.

• Facilitating communication among family members to enhance cooperation and support within the client's care process.

• Maintaining accurate electronic documentation of assessments, treatment plans, progress notes, and other relevant interactions.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
