Yearly Salary: $55K to $60K

Guided by our core values of Passion, Respect, and Excellence , the Social Worker Assistant supports the Social Services department in delivering compassionate, compliant, and resident-centered care.

Responsibilities for Social Worker Assistant:

• Assist the Social Services team in supporting residents with psychosocial, emotional, and practical needs.

• Help coordinate access to community resources, including healthcare services, welfare agencies, non-medical home care, and hospice services.

• Support implementation of departmental programs, goals, objectives, policies, and procedures.

• Maintain communication with residents, families, and interdisciplinary team members to support resident care planning.

• Assist with documentation, recordkeeping, and administrative tasks within the Social Services department.

• Support compliance with resident rights, safety standards, and regulatory requirements.

• Participate in maintaining a respectful, safe, and supportive environment for residents and staff.

• Perform other duties as assigned to support the Social Services department.

Qualifications for Social Worker Assistant:

• College degree preferred.

• Long-term care experience required.

• Ability to make independent decisions when appropriate.

• Knowledge of practices, procedures, laws, regulations, and guidelines governing post-acute care facilities.

• Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures.

• Competency in general social services functions, including patient rights, safety, and sanitation.

• Demonstrated integrity and professionalism in accordance with the company's Code of Conduct and completion of required compliance training.

• Knowledge of healthcare systems, welfare agencies, non-medical home care, hospice, and community social resources.
