-not applicable- Description Supportive Services Case Manager - Several Openings $24.75-$29.65/hour Position Summary: The Supportive Services Case Manager provides housing-focused case management and supportive services to residents of Permanent Supportive Housing (PSH) communities operated by WDC/HOC. Working within a Housing First framework, the Case Manager helps residents maintain housing stability, connects to healthcare and community resources, and achieves self-identified goals, while collaborating with property management and community partners. Reports to: Assistant Director of Community Development Essential Duties and Responsibilities:
Physical Demands: The physical demands and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. While performing the duties of this job, the employee is regularly required to stand; sit; use hands to finger, handle, or feel; reach with hands; and talk or hear. The employee must occasionally lift and/or move up to 10 pounds. Frequently required to walk and climb stairs. Must be able to drive; frequent travel to other locations such as resident sites, within the community and local region. Work is performed both in an office environment and on-site at residential units. The noise level in the work environment is usually moderate, and the environment is sometimes chaotic. Employees are occasionally exposed to outside weather conditions. Protocol for safety and security is a priority. Occasional work after normal business hours. Reports to: Assistant Director of Community Development Essential Duties and Responsibilities:
- Provide housing-focused case management to assigned residents.
- Complete assessments, develop individualized service plans, and maintain regular contact.
- Respond to referrals from property management, Coordinated Entry, community partners, and residents.
- Assist residents in resolving issues affecting housing stability, including lease compliance, rent arrears, behavioral health concerns, and neighbor conflicts.
- Connects residents to healthcare, behavioral health, substance use treatment, employment, education, benefits, and other community resources.
- Coordinate services with healthcare providers and community agencies.
- Provide crisis intervention and support using Housing First, trauma-informed, strengths-based, and harm reduction practices.
- Maintain accurate documentation, HMIS records, and required program reporting.
- Participate in case conferencing, multidisciplinary meetings, and quality improvement activities.
- Support resident engagement and perform other duties as assigned.
- Maintain a full caseload of assigned households.
- Complete required service contacts and documentation within established timelines.
- Support housing retention outcomes consistent with program and funder expectations.
- Maintain HMIS data quality standards and timely data entry.
- Assist residents in increasing income and accessing mainstream benefits whenever appropriate.
- Participate in quality improvement activities and contribute to program performance goals.
- Maintain required certifications and complete continuing education opportunities.
- Participate in professional development and networking opportunities.
- Participate in agency committees, trainings, and special projects.
- Associate degree in Social Work, Human Services, Psychology, Sociology, or a related field required; bachelor's degree preferred. Relevant experience may substitute for some education.
- 2-4 years' experience in case management, homelessness services, supportive housing, behavioral health, or related human services preferred.
- Experience with HMIS, Coordinated Entry, or HUD-funded programs is preferred.
- Valid driver's license and reliable transportation required.
- Knowledge of Housing First, trauma-informed care, motivational interviewing, ad harm reduction.
- Strong communication organization, crisis intervention, and problem-solving skills.
- Ability to work independently and collaboratively with residents, property management, healthcare providers, and community partners.
- Proficiency with Microsoft Office and HMIS or similar databases.
- Commitment to professionalism, confidentiality, and resident-centered services.
