Licensed Social Worker (LSW) - Flint, Michigan We are seeking a compassionate and skilled Licensed Social Worker (LSW) to join our dedicated team at Briarwood Nursing and Rehab in Flint, Michigan. As a valued member of our community, you will play a crucial role in providing high-quality social work services to our residents. About the Role As a Licensed Social Worker (LSW), you will work closely with our interdisciplinary team to assess, develop, and implement individualized care plans that promote the physical, emotional, and social well-being of our residents. You will: Conduct assessments and develop individualized care plans, ensuring that each resident receives personalized attention and support Provide counseling, education, and support services to residents and their families, addressing psychosocial and emotional challenges Develop and implement recreation and activity programs that foster social interaction, independence, and overall well-being Collaborate with other healthcare professionals to ensure seamless coordination of care and services Document resident progress, communicating effectively with family members and healthcare providers Requirements Valid Licensed Social Worker (LSW) certification in the state of Michigan Strong knowledge of social work principles, theories, and practices Excellent communication, interpersonal, and organizational skills Ability to work effectively in a fast-paced, dynamic environment Compassionate and empathetic approach to working with residents and their families #J-18808-Ljbffr Briarwood Nursing and Rehab
