Job Title: RN - Case Management - Travel


Location: Lexington, KY


Pay: $2,546.40 weekly


Shifts: Monday through Friday from 8 AM to 5 PM with some call


Seven Healthcare are seeking a dedicated RN Case Manager for a travel assignment in Lexington, KY. This role focuses on home health case management, requiring strong experience in comprehensive patient assessments and effective coordination of care transitions. Key Responsibilities of the RN - Case Management - Travel
  • Perform comprehensive assessments of high-risk members
  • Collaborate with primary care providers to implement comprehensive care plans
  • Identify and reassess cost-efficient levels of care
  • Coordinate non-medical support to ensure treatment compliance
  • Manage care transitions and coordinate with multidisciplinary teams
  • Engage specialty resources as needed
  • Maintain detailed records of clinical outcomes
  • Identify opportunities for health promotion and illness prevention
  • Act as patient advocate, ensuring privacy and confidentiality
Requirements of the RN - Case Management - Travel
  • Associates or Bachelor's Degree in Nursing or related field
  • Current, unrestricted Kentucky state Registered Nurse license
  • Ability to analyze complex medical information and make rational decisions
  • Experience with Home Health, OASIS, and HCHB
