Shifts: Monday through Friday from 8 AM to 5 PM with some call

Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement comprehensive care plans
• Identify and reassess cost-efficient levels of care
• Coordinate non-medical support to ensure treatment compliance
• Manage care transitions and coordinate with multidisciplinary teams
• Engage specialty resources as needed
• Maintain detailed records of clinical outcomes
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate, ensuring privacy and confidentiality

Requirements of the RN - Case Management - Travel

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted Kentucky state Registered Nurse license
• Ability to analyze complex medical information and make rational decisions
• Experience with Home Health, OASIS, and HCHB
