Follows patients throughout the continuum of care and ensures optimum utilization of resources. Provides ongoing support for discharge planning through comprehensive assessment of discharge needs to include barriers to compliance with recommended plan of care post discharge. Provides support and discharge planning for patients who are in need of ongoing medical/psychosocial care following discharge. In collaboration with other members of the healthcare team, assesses psychosocial needs of the patient and offers suggestions and facilitates the transition to follow-up care options. Qualifications High school diploma or equivalent. Bachelor’s degree preferred. CPI CPR One year experience in a psychiatric inpatient environment. #J-18808-Ljbffr Springstone
