Are you passionate about connecting people to the resources they need to thrive? The Delaware Division of Medicaid and Medical Assistance (DMMA) is looking for a compassionate, organized Health and Human Services Case Manager I to join its Newark team. In this entry-level case management role, you'll serve as a trusted advocate — helping individuals and families access the health, social, and community-based services that support their independence and well-being.

What You'll Do

Conduct comprehensive assessments to identify clients' needs and develop personalized service plans.
Coordinate with community partners and service providers to facilitate access to essential services.
Maintain detailed case records and documentation to ensure efficient, effective service delivery.
Advocate on behalf of clients to ensure their needs are met and their rights are protected.
Provide emotional support and guidance, empowering clients to achieve their goals.
Participate in team meetings and collaborate with colleagues to identify and implement process improvements.

What We're Looking For

Bachelor's degree in social work, human services, or a related field.
Experience in case management or direct service delivery preferred.
Strong communication and interpersonal skills, with the ability to build rapport and trust with diverse clients.
Demonstrated problem-solving skills and a commitment to high-quality, client-centered care.
Knowledge of community resources and social services programs.
Proficiency in relevant software and data management tools.
