Rehabilitation Case Manager II Drive Recovery & Discharge

Encompass Health is looking for a dedicated Case Manager for full-time, part-time, and PRN shifts based in Kentucky. In this role, you will coordinate and advocate for patients, ensuring high-quality outcomes from admission through to discharge. The ideal candidate has strong communication skills and is able to work independently. A degree in nursing or allied health is required, along with relevant certifications.
