Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and medical record reviews to evaluate the emotional, social, and environmental factors affecting each patient's recovery process.

• Provide compassionate counseling and emotional support to patients and their families, facilitating discussions around adjustment issues stemming from injuries, illnesses, or disabilities.

• Collaborate with the rehabilitation team to design and refine individualized treatment plans that address the unique psychosocial needs of each patient, contributing valuable insights based on assessments.

• Assist patients in connecting with community resources, including support groups, vocational rehabilitation services, and housing assistance programs, to ensure comprehensive post-rehabilitation support.

• Educate patients and their families about available community resources and potential post-rehabilitation services to ease transitions and enhance recovery experiences.

• Actively participate in discharge planning, coordinating with various service providers to effectively facilitate transitions back home, ensuring a smooth and supportive process for patients.

Qualifications\n

• A Master’s degree in Social Work (MSW) accredited by the Council on Social Work Education.
