Foster Care Intake Coordinator

Pennsylvania Mentor, a part of the Sevita family provides Residential and Treatment Foster Care services to youth who may be facing out-of-home placements, transitioning back into the family home or independent living alternatives, or involved in the juvenile justice system. The Intake Coordinator oversees and processes referrals, initiating timely communication with clinical and operations teams based on the needs of referred youth in the child welfare system, as well as at-risk youth. This ensures appropriate foster home, host home placements, and/or services are arranged to meet the youths' needs. Manage and accept referrals. Initiate appropriate communication with clinical and operations teams based on the needs of referred individuals to facilitate appropriate placement and/or services. Administer the referral process; work with the clinical and operations teams to facilitate individual placement and obtain appropriate services for individuals. Collect, verify, record, and process intake demographics, insurance/payment, and referral information. Create files for new individuals and ensure intake paperwork has been completed/requested. Input data and complete all placement paperwork, identifying problems and resolving issues that would prevent successful documentation/entry. Secure funding information and verify funding/insurance coverage arrangements. Complete all reporting requirements consistent with corporate and regulatory standards. Apply knowledge and understanding of programs, policies, procedures, and information to serve as a resource and assist others in resolving administrative issues within scope of authority. Assist with emergency coverage and crisis intervention support as needed. Qualifications Bachelor's Degree in Human Services or related field Two years of experience with foster care and/ or at risk youth preferred. Experience with admissions and intakes preferred Self-motivated and detail-oriented with ability to multi-task. Excellent communication and organizational skills.
