Your Role and Responsibilities

• Conduct thorough patient evaluations, order necessary diagnostic assessments, and craft personalized care strategies
• Diagnose and manage acute, chronic, or episodic conditions in partnership with fellow healthcare providers or independently (as per credentialing)
• Guide patients and their families on preventive health measures, wellness practices, and efficient disease management
• Prescribe necessary medications and treatment modalities within your professional scope
• Engage in patient follow-up and comprehensive chronic illness management
• Work in cohesion with interdisciplinary teams to guarantee integrated and holistic patient care
• Accurately document all patient interactions in electronic medical records (EMR)

Qualifications & Education

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Necessary Certifications

• Active Nurse Practitioner license in the state of California
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to acquire within a 90-day timeframe)
