Registered Nurse for Home Care Visits

Registered Nurse (RN)

We are seeking a compassionate and reliable Registered Nurse (RN) to perform periodic check-ups on patients receiving care in their homes. In this role, you will ensure the ongoing health and well-being of home care clients by conducting routine health assessments, monitoring chronic conditions, educating patients and caregivers, and working closely with the care team to deliver high-quality, person-centered care.

Responsibilities:

• Conduct routine wellness check-ups and health assessments at patients' homes
• Monitor and evaluate patient condition and vital signs
• Review and update care plans based on clinical findings
• Provide education and support to patients and families on managing health conditions
• Collaborate with physicians, home health aides, and care coordinators
• Maintain accurate documentation and timely reporting of visits
• Identify and report any changes in patient condition that require medical attention
• Ensure compliance with healthcare regulations and safety standards

Qualifications:

• Current and valid Registered Nurse (RN) license in NJ
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Reliable transportation and valid driver's license
• Ability to work independently and manage time effectively
• Familiarity with electronic medical records (EMR) systems preferred
