Qualifications: - Current and valid LPN license in the state of employment. - Proven experience in a clinical or home health setting is preferred. - Strong organizational and time-management skills. - Excellent interpersonal and communication abilities. - Ability to work independently and manage multiple patients efficiently. - Proficiency in using electronic health records (EHR) systems. - Valid driver's license and access to a reliable vehicle for travel to patient homes.
