Community Outreach Patient Navigator
St Joseph Hospital & Medical Center
Saint Joseph’s Hospital and Medical Center is seeking an Outreach Patient Navigator to connect patients to essential services and coordinate care within the community. You will engage with events, deliver educational presentations, and build partnerships to extend access to care for HIV, HBV, HCV, and syphilis-affected populations.
