This role is essential in improving health outcomes by providing education, resources, and guidance for underserved populations, including adults with Serious Mental Illness, uninsured, underinsured, and vulnerable community members. The Health Navigator collaborates with healthcare providers, case managers, and external community resources to promote patient-centered care and facilitate access to essential health services. The Health Navigator will provide BHT level behavioral health and skills interventions to facilitate overall wellness and health to patients served.
