Job Title: Physical Therapy Assistant (PTA)

Setting: Skilled Nursing Facility (SNF)

Position Overview:
We are seeking a dedicated Physical Therapy Assistant (PTA) for a Skilled Nursing Facility assignment. Candidates with SNF experience and NetHealth/Rehab Optima knowledge are highly preferred.

Responsibilities:
Assist Physical Therapists in implementing individualized treatment plans
Provide therapeutic exercises and mobility training to patients
Monitor patient progress and report changes to the supervising Physical Therapist
Assist patients with improving strength balance mobility and functional independence
Maintain accurate and timely patient documentation
Utilize NetHealth/Rehab Optima EMR system for charting and treatment updates
Educate patients and caregivers on therapy exercises and rehabilitation goals
Ensure compliance with facility policies safety standards and therapy protocols

Requirements:
Active California PTA license required
Minimum 1 year of Physical Therapy Assistant experience preferred
Skilled Nursing Facility (SNF) experience preferred
Experience with NetHealth/Rehab Optima strongly preferred
Strong communication and patient care skills required
Ability to work effectively within an interdisciplinary rehabilitation team
