Responsibilities
- Perform initial assessments and develop individualized treatment plans for residents.
- Provide skilled physical therapy interventions focused on mobility, gait, balance, functional mobility, and activities of daily living.
- Reassess and adjust treatment plans based on resident progress and goals.
- Document evaluation findings, plan of care, progress notes, and discharge summaries in the resident chart per regulatory and facility policies.
- Collaborate with nursing, therapy aides, and other care team members to coordinate care and support discharge planning.
- Educate residents and families on home programs and safety strategies to promote continued progress.

Requirements
- Active New York state Physical Therapist license
- BLS certification
- Prior experience in a skilled nursing facility
- Weekend availability, specifically Saturdays (Saturdays only required)
- Ability to work effectively within a skilled nursing facility setting and follow established policies and procedures
