Provide education and resources to patients,cliniciansand community partnersonavailable care options. Support patients asthenavigate the healthcare continuum. Providepersonalized advice based on the patient’s unique goals and care needs. Provide guidance to patients during challenging situations and decision‑making processes. Assistwith advance care planning tailored to a patient's wishes and values. Educate and guide patientsregardinghealthcare systems, including home health, hospice, palliative care, hospitalizations, and skilled nursing or rehabilitation placement. Maintaina strong senseof responsibility in responding to patients and clinicians, ensuringtimelyand effective support is provided as needed. Ability to maintain careful records and document all patient/family interactions. Qualifications High school graduate or equivalent required. College degree preferred. 3 years’ experience in healthcare office, preferably in hospice and/or home health care. Proficient computer skills required Demonstrates excellence in customer service and time management Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices. Demonstrates good communications, negotiation, and public relations skills. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
