Physical Therapist

We are growing and need additional staff to provide in home services in the fastest growing segment of community health - Homecare and Hospice! Clinical assessment and treatment of patients referred by a licensed physician. Coordinate and communicate treatment activities with health care team members and the patient. Will also supervise Physical Therapy Assistant and contribute to the professional development of new staff and health professional students. May be scheduled to work during the weekend, if working a Saturday a day off during the week prior to the Saturday, if working on Sunday, will be given a day off during the week following. Education and Licensure Required: Master's Degree or Doctorate of Physical Therapy Must be licensed in NH and VT Valid Driver's License and agency level auto insurance Skills and Abilities: Evaluates patient's need for physical therapy and develops appropriate treatment plan. Provide appropriate care and treatment to patients based on physician care plan, agency policy and professional standards of care. Instructs family members, PTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques. Coordinates physical therapy progress with other caregivers and ensures appropriate physical therapy techniques. Refers patient to other professionals or agencies when appropriate.
