Heart Failure Nurse Practitioner Opportunity

This is an exceptional opportunity for an experienced Adult-Gerontology Nurse Practitioner (AGNP) or Acute Care Nurse Practitioner (ACNP) to join our dedicated Heart Failure Center team at Ascension Saint Agnes, delivering comprehensive, evidence-based care within a supportive and highly collaborative cardiology department.

Join an established team of 10 physicians and 7 APPs.

Schedule: (4 x 10-hour shifts per week), MondayFriday. No weekends or holidays required.

Call structure: None.

Practice setting: Outpatient.

Employed position within a nonprofit health system.

ANCC or AANP Board Certification.

Eligible for state medical licensure and DEA registration.

Perform comprehensive clinical assessmentsincluding volume status evaluations and outpatient IV diuretic managementto optimize guideline-directed medical therapy (GDMT) for complex heart failure patients.

Coordinate seamless post-discharge follow-up care and interdisciplinary treatment plans to strengthen continuity of care and prevent hospital readmissions.

Serve as a clinical leader and mentor for Heart Failure Center RNs, collaborating closely with community cardiologists, pharmacists, and care coordinators to execute individualized patient care plans.

What minimum requirements you'll need

Licensure / Certification / Registration:

• Nurse Practitioner with Prescriptive Authority credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:

• Master's degree of Nursing required.
