Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Collaborate with other healthcare professionals to develop and implement individualized care plans

• Maintain accurate and up-to-date records of patient care and treatment

• Participate in care conferences and multidisciplinary team meetings to ensure comprehensive care

• Assist with daily living activities, such as grooming, bathing, and transferring patients

Requirements:

• Current LPN license in the state of Massachusetts

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills

• Strong attention to detail and ability to prioritize tasks
