Registered Nurse Care Coordinator
Location: UCHealth Aurora, CO
Work Schedule: Full Time, 80.00 hours per pay period (2 weeks)
Shift: Days
Pay: $38.91 - $60.31 / hour. Pay is dependent on applicant's relevant experience
This is a hybrid position - approximately 50% remote and 50% onsite as work dictates. Summary: Coordinates and manages care for patients with chronic or complex conditions to improve outcomes, ensure continuity of care, and optimize resource utilization.
Responsibilities: Assesses patient needs and develops individualized care plans in accordance with NCQA standards. Partners with patients to evaluate healthcare needs, lifestyle choices, and resource utilization impacting health outcomes. Educates, coaches, and advocates for patients and caregivers regarding treatment options, disease management, medications, and resources. Coordinates care across the continuum, facilitates transitions, and manages assigned patient populations to reduce gaps in care and readmissions. Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
Requirements:
- Registered Nurse
- Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years' experience.
- Preferred: Bachelor's degree in Nursing.
- Registered Nurse (RN) license that is valid in the state of practice or a current multistate RN license in accordance with the Nurse Licensure Compact (NLC).
- 1 year of Clinical nursing experience and care management experience.
