Wage Range: $134,326 - $214,926 per year Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity. Our Team Physicians - The patient’s Attending Physician continues to provide oversight of care and Hospice Physicians consult with hospice staff and the attending physician. Nurses (RN or LPN) Social Workers Certified Nursing Assistants (Home Health Aides) Spiritual Care (Chaplains) Pharmacists Trained Volunteers – offer companionship for patients and respite for families Job Summary Role will manage a multi-disciplinary team in a designated geographical area comprised of RN's, LPN's, Social Workers and Home Health Aide/CNAs. Role will assume 24-hour accountability and provision of quality of care and service for patients at end of life. Responsible for planning, developing, organizing, integrating, and overseeing programs, staff, and budget in support of the organizational goals and commitments. Primary Duties Responsible for service line(s) to include, but not limited to, Medical and Psychiatric nursing, rehabilitative and social work services. Ensures that operations follow established policies and procedures and comply with all applicable standards and regulations including licensure, Medicare Conditions of Participation, JC, OSHA regulations and accepted standards of practice. Ensures, promotes, and assesses compliance with regulatory agency standards and acts as a resource to staff to answer regulation-based questions. Facilitates patient care and operations including organization and management within established policies, procedures, standards of care, budgetary parameters and strategic objectives. Performs other duties as assigned.
