Unlicensed Medical Social Worker - Crestview Rehabilitation and Nursing Facility

Job Summary The Medical Social Worker Unlicensed collaborates with the Clinical Care Coordinator (C3) and other members of the care management and health care teams, utilizes professional Social Work skills in accordance with social work practice standards to: assist the patient in achieving optimal health; assure appropriate use of health care resources; facilitate/coordinate timely care along the continuum; decrease unnecessary variances in care through proactive care management; develop a safe and timely discharge plan in accordance with the Joint Commission and CMS Guidelines. Each patient will be ensured a planned program of continuing care which meets his/her discharge needs and to ensure a smooth hospital stay with a seamless transition to the next level of care following discharge from GHS. Qualifications Master's Degree in Social Work is required. Work experience in health care or long‑term care is preferred. Strongly preferred: Good working knowledge of the dynamics of social work intervention. Some knowledge of social welfare policies and services. The basic ability to assess residents and his/her interaction with social systems and processes of which he/she is a part. Basic knowledge of the concepts and principles of social work's practice and ethics of the profession.
