Care Coordinator

Role Overview: This position involves serving as a primary contact for families of children with developmental, physical, or severe emotional disabilities, supporting their access to community-based services through assessment, planning, coordination, and ongoing support. Key Responsibilities Conduct functional assessments and develop individualized service plans with families and providers Authorize, coordinate, and monitor services in alignment with plans Facilitate family-centered team meetings and maintain accurate documentation Collaborate with service teams and supervisors to determine interventions Testify at legal proceedings when necessary Participate in staff development, supervision, and training Direct Service Expectations Promote client independence and growth through trauma-informed, family-centered services Utilize evidence-based practices supported by organizational and funding guidelines Accurately complete time reporting Qualifications & Skills Bachelor’s degree in a health or human services field At least 1 year of experience working with children in the target population Strong communication, documentation, and organizational skills Proficiency with electronic health records and computer applications Valid driver’s license and reliable transportation Bilingual fluency in Spanish is preferred but not required Additional Information: The role involves community-based work with local travel, occasional overnight stays, and responding effectively in crisis situations.
