Family Nurse Practitioner (FNP)

Our client, a respected regional healthcare organization, is seeking a compassionate and dedicated Family Nurse Practitioner to join its outpatient primary care team. Preventive Care Electronic Medical Records (EMR) Clinical Documentation Chronic Disease Management Primary Care Responsibilities Provide comprehensive primary care services across all stages of life in an outpatient setting. Diagnose, treat, and manage acute and chronic health conditions. Deliver preventive care, wellness visits, health education, and chronic disease management. Develop long-term relationships with patients and families through continuity of care. Collaborate closely with physicians, advanced practice providers, nursing staff, and other healthcare professionals. Maintain accurate clinical documentation using electronic medical records. Participate in quality improvement initiatives and promote evidence-based patient care. Qualifications Licensed Family Nurse Practitioner (FNP). Must hold or be eligible for an RN license in the state of New York. Experience in primary care, family medicine, internal medicine, or community health is preferred.
