Housing Case Manager

About the Position: The Housing Case Manager supports individuals and families who are experiencing or at risk of homelessness by providing case management, housing assistance, and connections to community resources. This position conducts client assessments, coordinates services, supports long-term housing stability, maintains HMIS records, and collaborates with service providers and community partners. Ideal Candidate: The ideal candidate is a compassionate and resourceful professional with experience in case management, homeless services, or housing programs. They can build trust with clients, manage a demanding caseload, respond appropriately to crises, and collaborate effectively with community partners. Familiarity with HMIS, HUD regulations, and housing and social-service resources is highly desirable. Day in the life consists of: Conducting client intake interviews and assessing housing and supportive-service needs. Developing individualized service plans and connecting clients with appropriate resources. Supporting clients with maintaining long-term housing stability. Coordinating referrals to housing programs, rental assistance, shelters, and community-based services. Maintaining accurate case notes, client records, and service outcomes in HMIS. Collaborating with outreach teams, service providers, law enforcement, and community partners. Following up with clients and partner agencies to monitor progress and address barriers. Participating in outreach activities, case conferences, community meetings, and presentations. Assisting formerly homeless clients with maintaining housing, complying with lease requirements, and addressing issues that may place their housing at risk. Supporting coordinated-entry activities, including referrals, data collection, and program documentation. Licenses, Certifications, and Equipment: Possession of a valid California Driver’s License, which must be maintained throughout employment. Desirable Qualifications Experience working with individuals and families experiencing homelessness, including those with mental health conditions, substance-use disorders, chronic health conditions, or histories of trauma. Knowledge of Housing First, trauma-informed care, harm reduction, motivational interviewing, and person-centered service-delivery principles. Familiarity with the Coordinated Entry System (CES), Homeless Management Information System (HMIS), and Continuum of Care (CoC) processes. Knowledge of federal, state, and local homelessness programs, including HUD-funded programs such as the Emergency Solutions Grant (ESG), Continuum of Care (CoC), and Community Development Block Grant (CDBG). Experience developing individualized housing-stability plans, conducting client assessments, and coordinating referrals to medical, behavioral health, employment, income, and supportive services. Experience collaborating with community-based organizations, healthcare providers, behavioral health agencies, veterans’ services, landlords, property managers, and government partners. Knowledge of fair-housing laws, landlord-tenant relations, and community housing resources. Experience preparing case notes, maintaining confidential records, and documenting services in HMIS or a comparable case-management system. Bilingual proficiency in Spanish. Desirable Certifications and Training: Certified Case Manager (CCM) certification. Mental Health First Aid certification. Motivational Interviewing training. Trauma-Informed Care training or certification. CPR and First Aid certification. Knowledge of and Ability to: Apply case-management and housing-stability practices for individuals and families experiencing or at risk of homelessness. Understand federal, state, and local housing and social-service programs, HUD regulations, and coordinated-entry processes. Conduct empathetic, client-centered interviews, intake assessments, and service planning. Connect clients with housing assistance, supportive services, and community resources. Build trust and maintain professional relationships with clients, service providers, and community partners. Handle crisis situations and determine appropriate interventions or referrals. Maintain professional boundaries, client confidentiality, accurate case files, and HMIS records. Interpret and explain housing-program rules, policies, regulations, and eligibility requirements. Communicate clearly and effectively, both verbally and in writing. Work independently, exercise sound judgment, manage a heavy caseload, and meet established deadlines. Collaborate with City staff, law enforcement, social-service professionals, and community organizations. Represent the City professionally at outreach events, public meetings, and community presentations. Physical Demands and Working Conditions: This position involves work in both an office environment and outdoors in the field. The Housing Case Manager conducts outreach activities and may work in areas inhabited by individuals experiencing homelessness. Physical Demands: Sit for extended periods while using a computer, keyboard, and related office equipment. Stand, walk, reach, bend, twist, kneel, and grasp. Walk on slippery or uneven surfaces and slopes while working in the field. Crouch, twist, and climb ladders or stairs as required. Lift and move items weighing up to 30 pounds. Possess the mobility, balance, coordination, vision, hearing, and dexterity necessary to perform the position’s assigned duties. Environmental Conditions: Work is performed indoors and outdoors. Office noise levels are generally quiet to moderate, while field assignments may involve exposure to loud noise. Fieldwork may involve exposure to heat, changing weather conditions, road hazards, vibration, construction materials, dust, hazardous substances, fumes, blood or other bodily fluids, and human-borne parasites. Outreach activities require working in areas inhabited by individuals experiencing homelessness, including unclean environments and interactions involving challenging situations or individuals. Working Conditions: City employees may be required to work overtime, weekends, evenings, and holidays to accommodate the City’s operational needs. Employees are also designated as Disaster Service Workers pursuant to California Government Code Sections 3100-3109.
