Wellness Navigator

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient-centered medical homes, medication‑assisted treatment, and crisis services throughout Arizona. This role supports clients with recovery and treatment for opioid use disorder, homelessness, and mental illness. Job Summary The Wellness Navigator will help to remove personal and environmental obstacles to health care access. The Navigator works closely with primary medical providers, the clinical team, and staff of other agencies involved in the patient's care to proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy, and support primarily in‑home and/or in the community. The Navigator engages, educates, and offers support to individuals, their families, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow‑up. Skills / Requirements High school diploma or GED required; associate’s degree (or higher) in a field related to behavioral health preferred. Education and/or experience in the field of health and nutrition preferred. Or equivalent combination of education and experience. Current AZ driver’s license. Clean 39‑month MVR. Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures required. Arizona fingerprint clearance card required.
