Role Overview: A licensed Physical Therapist providing personalized, one-on-one care to older adults in their homes or senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments. Key Responsibilities Conduct evaluations, develop individualized care plans, and deliver therapy sessions to support wellness and functional independence. Document patient progress accurately within electronic medical records. Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid physical therapy license or eligibility in practice state(s). Degree from an accredited PT program. Proficiency with computers and EMR systems. Current CPR certification. Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, and drive between locations, with proficiency in repositioning patients and performing therapeutic activities.
