Psychiatric Mental Health Nurse Practitioner – Old Town Clinic – Central City Concern Practice psychiatry where it matters most. This is an opportunity to provide life‑changing care while working alongside primary care, behavioral health, addiction medicine, pharmacy, and case management professionals. What You’ll Do Provide psychiatric evaluations and medication management Treat co‑occurring mental health and substance use disorders Prescribe and manage medications for opioid use disorder and recovery support Deliver crisis intervention and risk assessments Collaborate within a multidisciplinary, integrated care team Utilize trauma‑informed and harm reduction approaches to care Why Central City Concern Mission‑driven work with meaningful community impact Integrated care model recognized for innovation Collaborative and supportive clinical teams Opportunity to work with complex, rewarding patient populations Help people build healthy, housed, resilient, and engaged lives Schedule: Monday‑Friday 8:00am‑5:00pm Location: Old Town Clinic Compensation: $164,000‑$213,000 annually (Range varies based on professional experience and licensure/certifications) Minimum Qualifications Bachelor’s degree in nursing and Master’s degree in Psychiatric/Mental Health Nursing from state board‑approved schools of nursing Current license to practice nursing in Oregon and certified as a PMHNP by the Oregon State Board of Nursing and national certification Must have full prescriptive powers in Oregon with a DEA number Practicing as a PMHNP for 3 or more years highly preferred Must meet CCC privileging requirements as required by FTCA Current BLS certification required prior to start Bilingual in Spanish preferred Must meet applicable training requirements for Buprenorphine prescribing Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation Must pass a pre‑employment drug screen, TB test, and background check (including clearance by the DHS Background Check Unit) The OAR prohibits SUDS program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act Must be able to work within an integrated, multidisciplinary setting Must adhere to agency’s non‑discrimination policies Ability to adhere to Central City Concern’s drug‑free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug‑Free Work Place Act of 1988 Ability to effectively interact with co‑workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life styles, and sexual orientations, treating each individual with respect and dignity Essential Duties and Responsibilities Work as a member of the Old Town Clinic multidisciplinary team to deliver comprehensive, client‑centered and trauma‑informed integrated services Actively participate in the integration of services at Old Town Clinic and identify new opportunities to improve collaboration between and within service and care teams while carefully following information‑sharing guidelines Proactively identify opportunities for improvement within Old Town Clinic workflows and logistics; participate in review of key performance indicators and continuous improvement plans Build understanding of and appreciation for diversity among Old Town Clinic clients and support their connection to culturally responsive and culturally specific services as requested Provide direct patient care including psychiatric evaluation, consultation, psychopharmacological and psychotherapeutic interventions, and monitoring and addressing adverse effects Prescribe and manage buprenorphine, Vivitrol, Naltrexone, and Acamprosate Document plan for medical and psychiatric treatment and ongoing treatment in the outpatient setting Provide crisis intervention, suicide assessment and intervention Conduct care of psychiatric/mental health emergencies and initiate arrangements for continuing care or referral to appropriate facility Provide brief psychotherapy interventions including motivational interviewing Provide patient and family education Provide clinical consultation to staff in behavioral and primary care disciplines, including consultation to other CCC SUDS practitioners Participate in clinical staff meetings and case conferences to utilize interdisciplinary expertise and facilitate individual treatment plans for clients Refer patients, as necessary, to other mental health care providers and community resources/services Participate in records review to ensure that services are clinically appropriate and effective Maintain client records as required by program standards and funding requirements Participate in compliance, quality assurance and quality improvement activities as directed Participate in training opportunities to advance skills and knowledge in behavioral and primary care Work within scope of training and experience and seek consultation when appropriate Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies regarding confidentiality, privacy, and security Attend all mandatory CCC trainings in a timely manner Perform other duties as assigned Skills and Abilities Consider the impacts and outcomes for underserved communities during decision‑making process Consider impacts of oppression, structural racism, and individual bias on client outcomes Knowledge of evaluation and treatment planning for clients with substance use disorders, including both pharmacotherapies and psychotherapies Knowledge of evaluation plans and treatment of clients with a variety of psychiatric disorders, with psychopharmacological and psychotherapeutic interventions Skills in providing patient care, including rapidly building rapport Make treatment decisions based on assessment of patients concerns within PMHNP scope of practice Provide non‑judgmental and compassionate care to vulnerable, complex populations and seek supervision as necessary Practice independently and seek consultation when appropriate Delegate patient care, as needed, and if appropriate Work in a busy clinic setting and respondively attend to unpredictable requests for assistance from colleagues Interact successfully with a diverse population, including both patients and co‑workers Sufficient manual dexterity and physical ability to perform assigned tasks Maintain accurate records and necessary paperwork Remain calm and composed when dealing with clients who are in crisis Maintain strict confidentiality of patient information Excellent communication skills including diplomacy, tact, and bedside manner Benefits at Central City Concern Generous paid time off plan beginning at 4 weeks of PTO accrual per year (accrual amount/rate increases with longevity) Up to 11 recognized holidays and 2 personal holidays (dependent on work shift schedule) 403(b) retirement savings plan with an employer match of 4.25% in year one, 6% in year two, and 8% in year three Comprehensive medical, vision, and dental insurance coverage Employer‑paid life, short‑term disability, and long‑term disability insurance Sabbatical program offering extended time off at years 7, 14, and 21 We offer an extensive total rewards package, competitive compensation, student loan repayment options, and retirement savings matching plans.
