Associate Community Health Worker (CHW)

Join a dedicated healthcare team in the heart of the Midwest as an Associate Community Health Worker (CHW). Under the guidance of the Director of Care Management, you will play a vital role in helping patients access and navigate essential community services. This position focuses on outreach, providing support and information to community members, and assisting with data collection to identify health needs and improve processes, especially for high-risk populations.

Essential Duties and Responsibilities

• Build and maintain strong relationships with community partners, patients, and their families while promoting healthy behaviors.
• Assist families, groups, and communities in accessing social and health-related services.
• Assess client needs and refer them to appropriate community resources and agencies, developing processes to evaluate Social Determinants of Health in various settings.
• Participate in community meetings and events such as health fairs to stay informed about community issues and share resource information.
• Advocate for individual and community health needs.
• Serve as a liaison between healthcare providers, community organizations, and the populations served.
• Develop referral and follow-up processes to address patient needs effectively.
• Distribute educational materials tailored to the target population.
• Perform other duties as needed to support organizational goals.

Education

Associate degree in any field is required.

Experience

Experience with social services or public health systems is preferred.

Requirements

• Obtain Basic Life Support (BLS) certification within the first 90 days of employment (training provided).
• Earn Community Health Worker certification within the first six months of employment.

Mental and Physical Demands

This role requires considerable mental focus with frequent interruptions. Light lifting up to 15 lbs., standing, sitting, walking, reading, and speaking are part of daily activities.

Working Conditions

Typical office environment with some noise and distractions. Possible eye strain from computer use. Attendance at public meetings is required. There may be exposure to infectious or communicable diseases when interacting with patients in various settings.
