Care Ally Case Manager - RN/LPN/Social Worker (Hybrid)

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary team. You will assess member health, develop and coordinate a plan of care, and liaise with RP/POAs, PCPs, and care teams to ensure high-quality care in senior living settings. The position requires autonomous work with travel 2–3 days per week and the ability to work from a home office for the remainder.
