Occupational Therapist (OT) - Home Health

Role Overview: This Occupational Therapist evaluates and develops treatment plans for patients of all ages to improve or adjust daily living activities. They collaborate with the healthcare team, modify care plans as needed, and serve as a clinical resource and role model for staff and students. The therapist provides services in patients' homes within designated areas. Key Responsibilities Review and communicate care plans with physicians; document thoroughly and accurately. Educate patients and families, overcoming barriers to understanding. Update goals and document progress, ensuring timely and compliant record-keeping. Coordinate case conferences and communicate with interdisciplinary team members. Order supplies, plan discharges, and ensure comprehensive discharge documentation and planning. Evaluate patient function, monitor reactions to treatments, and report changes. Supervise Occupational Therapy Assistants and Home Health Aides, including supervisory visits. Complete assessments such as OASIS, evaluate home safety, and recommend equipment. Manage appointment scheduling and assess patients’ homebound status and safety. Qualifications & Skills Bachelor’s Degree in Occupational Therapy. Licensed Occupational Therapist (IDFPR), BLS certification, valid Illinois driver’s license, and auto insurance. Ability to work collaboratively within an interdisciplinary team and maintain reliable transportation. Strong assessment, documentation, communication, and patient education skills. #J-18808-Ljbffr
