The CSS works collaboratively, providing measurable interventions to help families assimilate into their community. Job Responsibilities Assist children and families access community support services: education, medical, social, vocational, recreational, therapeutic, and other services. Assess strengths and service needs of children and families. Develop measurable, individualized interventions and track progress. Provide transportation for families safely and responsibly. Direct services in home, school, and community. Coordinate child‑focused social services building on family and community strengths. Provide crisis intervention as needed within role scope. Maintain an up‑to‑date community support resource manual. Implement service interventions based on the ISP and document in Evolv. Use evidence‑based practices when possible and complete required documentation for each child and family. Demonstrate flexibility in scheduling, including evenings and weekends. Attend weekly staff meetings. Collaborate with supervisor, therapist, and MCHS staff to monitor progress. Assist with after‑care planning to ensure referrals, appointments, and services are available. Participate in telehealth/telepsychiatry sessions during scheduled times. Schedule medical appointments for families as needed. Relate changes in medical information, including medication changes, to the PMHC nurse. Ensure new psychotropic medications have guardian release signed. Perform other duties as assigned. Required Qualifications Bachelor’s degree in social work or related field and at least two years of direct experience working with children and families. Must meet Mississippi Department of Mental Health certification requirements for a Community Support Specialist. Demonstrated competence in implementing evidence‑based and best practices with children and families. Competence in completing payment‑related reports. Computer skills required. Availability for flexible schedule, including evenings and weekends. At least 21 years old, valid Mississippi driver’s license, good driving record, proof of auto liability insurance, and willingness to use personal vehicle for agency business. #J-18808-Ljbffr
