Patient Care Coordinator
We are seeking a dedicated and skilled RN to serve as a Patient Care Coordinator. The role requires a minimum of one year of experience in case management or as a patient care coordinator. Candidates must possess an active Oregon or Washington RN license or a compact license. Experience with EPIC charting, Medicaid, and Medicare is highly preferred. This position involves working weekends and holidays. RNs will be expected to float to at least one neighboring location as needed. The ideal candidate will demonstrate strong patient advocacy skills and a commitment to high-quality patient care.
