Key Responsibilities Case Management & Advocacy: Oversee the development, implementation, and ongoing monitoring of Individual Support Plans (ISPs). Ensure these plans accurately reflect the individual's needs, preferences, and long‑term goals. Regulatory Compliance: Maintain thorough, up‑to‑date documentation in accordance with Pennsylvania ODP regulations and agency policies. Prepare for and participate in licensing audits and internal quality reviews. Stakeholder Collaboration: Serve as a central point of contact for families, support coordinators, direct support professionals, and medical providers to ensure coordinated care. Crisis Intervention & Support: Respond promptly to critical incidents, ensuring the safety and well‑being of the individuals we serve. Facilitate proactive problem‑solving to mitigate ongoing risks. Team Leadership: Provide guidance and support to direct care staff regarding implementation of support plans and best practices for IDD care. Qualifications Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a closely related field. Experience: Minimum of 2–3 years of experience in human services, specifically working within the IDD population and the Pennsylvania ODP system. Technical Skills: Proficiency in HCSIS (Home and Community Services Information System) and standard electronic record‑keeping software. Soft Skills: Strong communication, organizational, and conflict‑resolution skills. A warm, patient, and professional demeanor is essential.
