Case Manager RN (Care Coordinator)

Location: Tallahassee, FL

Type: Full-Time

Pay: $31.94/hr – $43.92/hr

Shift & Schedule: Day Shift | 8:00 AM – 4:30 PM EST | 1 Weekend Every 4 Weeks

Position Overview

We are seeking an experienced Case Manager RN / Care Coordinator to support interdisciplinary care planning, utilization management, discharge coordination, and resource management within an acute care setting. This role focuses on improving patient outcomes, optimizing resource utilization, and ensuring efficient transitions of care while collaborating closely with physicians, nursing staff, and community resources.

Responsibilities

• Perform comprehensive medical and psychosocial assessments for assigned patients.

• Develop, document, and communicate individualized case management plans of care.

• Conduct medical necessity and utilization reviews using approved clinical criteria.

• Review admission status and level of care appropriateness.

• Coordinate discharge planning and facilitate safe patient transitions.

• Identify barriers to care and implement solutions to improve patient throughput.

• Lead interdisciplinary care team meetings and collaborate with healthcare providers.

• Communicate clinical information to insurance providers and third-party payers.

• Advocate for patients and families while supporting quality care initiatives.

• Monitor compliance with regulatory requirements and organizational policies.

• Identify opportunities for cost containment while maintaining quality outcomes.

• Maintain accurate documentation and case management records.

Required Qualifications

• Associate Degree in Nursing (ADN) or RN Diploma required.

• Active Florida RN License or Compact RN License.

• Minimum 3 years of recent acute care Case Management experience

OR

• Minimum 3 years of acute care RN experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or Emergency Department

OR

• Home Health/Insurance Case Management experience combined with acute care nursing experience as outlined above.

• Strong assessment, discharge planning, and care coordination skills.

• Excellent communication and interdisciplinary collaboration abilities.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).

• Case Management, Utilization Review, or Nursing Certification.

• Acute Care Hospital Case Management experience.

• Experience with utilization review and insurance authorization processes.
