Medical City Heart & Spine Hospitals in Dallas, TX is seeking an RN Case Manager for a full-time day shift. You will coordinate care, assess discharge needs, and collaborate with the multidisciplinary team to ensure efficient patient transitions. The role requires TX RN licensure, an ADN or nursing diploma (BSN preferred), and 3+ years of hospital nursing or 2+ years in case management.
