Hospice RN Case Manager - Compassionate Home Care Leader
Transitions Care
Transitions Hospice is seeking an RN Case Manager in Ottawa, IL to coordinate care and guide the interdisciplinary team. You will be the clinical anchor for patients and families, aligning plans with physicians and care teams to ensure compassionate, comprehensive end-of-life care. The role requires an active RN license in Illinois, at least 1 year of hospice or home health experience, and strong clinical judgment for independent field work.
