Physical Therapist Requirements

• Current Physical Therapist license valid in Kentucky
  
• Current CPR certification
  
• Two to three years of relevant experience
  
• Outpatient physical therapy experience
  
• Strong patient-assessment and treatment-planning skills
  
• Experience providing therapeutic exercises and manual therapy
  
• Ability to maintain accurate clinical documentation
  
• Effective communication and interdisciplinary teamwork skills
  
• Compassionate and patient-focused approach to care
  

Key Responsibilities

• Provide outpatient rehabilitation services according to physician orders and clinical standards.
  
• Evaluate patients and develop individualized plans of care.
  
• Perform therapeutic exercises and manual-therapy interventions.
  
• Provide mobility and functional training using adaptive equipment.
  
• Monitor patient progress and adjust treatment plans as appropriate.
  
• Educate patients and families regarding exercises, mobility, and continued care.
  
• Collaborate with physicians and other healthcare professionals.
  
• Maintain accurate and timely documentation using the appropriate clinical software.
  
• Participate in care planning and discharge planning.
  
• Follow all patient-safety, infection-control, and facility procedures.
