Licensed Practical Nurse (LPN) – Rehabilitation Shift: Full Time | Nights Compensation: $8,000 sign on bonus About the Team Ascension Saint Thomas Rehabilitation Hospital is a 40‑bed acute rehabilitation hospital located in Nashville, TN, and part of Lifepoint Health – a nationwide network delivering acute care, rehabilitation, and behavioral health services. Our mission is to provide exceptional patient-focused care and enable recovery. Responsibilities Administer medications safely, accurately, and based on patient age and clinical condition. Minimize potential for medication errors by following proper medication administration procedures. Assist the RN with initial assessments and patient reassessments per policy. Identify and implement appropriate rehabilitation nursing interventions. Perform timely and accurate Quality Improvement (QI) assessments under RN supervision. Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly. Assess and reassess pain; use appropriate pain management techniques and educate patients and families. Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors. Develop and implement individualized teaching plans based on patient and family learning needs. Perform other duties as assigned. Qualifications Current state LPN license from an accredited nursing school. Graduate of an accredited Licensed Practical/Vocational Nurse program. Minimum six months of Med/Surg experience in an acute care setting preferred. CPR/BLS certification required; IV certification highly preferred or obtained within 6 months. Strong clinical skills and attention to detail. Compassionate approach and commitment to patient-centered care. Ability to work effectively within an interdisciplinary team.
