Job Description Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians that considers the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient. Reassesses patients at appropriate intervals, documents progress, and determines the effectiveness of therapy. Observes and records patient activities and findings, reporting any changes or deviations in the plan of care to the physician. Instructs patients and caregivers on the use of assistive devices and establishes home programs. Documents treatments accurately and timely in medical records and updates care plans as necessary. Schedule Full time days. Monday-Friday 8:30am-5:00pm with rotating call. Location 9961 W Colonial Dr, Ocoee, FL 34761. Coverage Area Clermont, Minneola, Mascotte, Groveland and Leesburg. Home Care Regulations and Third-Party Reimbursement as it impacts care delivery. Demonstrated knowledge of physical therapy theory, modalities, treatments, and skills. Basic computer skills and knowledge of Microsoft Office. Must be able to read, write, and speak conversational English.
