Care Manager RN – Providence Saint John’s Health Center Providence Saint John’s Health Center in Santa Monica, CA. This per diem position offers 8‑hour day shifts. Providence Saint John’s is a Magnet®‑recognized hospital, known for high‑quality care. The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient‑centric care management for at‑risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. The goal is right care at the right time in the right setting. Responsibilities Assess and plan individualized care for at‑risk patients. Coordinate and monitor care across care teams. Manage resource utilization and perform utilization review. Plan discharge and facilitate transitions. Document interventions and maintain regulatory compliance. Advocate for patient needs and preferences. Required Qualifications Associate’s Degree in Nursing (diploma or degree) upon hire. California Registered Nurse License upon hire. At least 2 years of acute‑care experience in medical‑surgical or inpatient setting. One year of experience in care management or utilization review, or successful completion of the TIPS or Case Management Orientation Program. Preferred Qualifications Bachelor’s Degree in Nursing or higher within 3 years of hire. Case management certification (ACM or CCM).
