Pride Health is hiring a Travel Registered Nurse (RN) to support our client's Home Health team in Indianapolis, IN . If you have Home Health, OASIS, and Homecare Homebase (HCHB) experience, we'd love to hear from you!

Job Details

• Job Title: Travel Registered Nurse (RN)
• Location: Marion, Hamilton & Johnson Counties, IN
• Setting: Home Health
• Schedule: Monday–Friday | 8:00 AM – 4:30 PM
• Weekly Hours: 40 Hours Guaranteed
• Productivity: 25–30 Points/Week
• On-Call/Weekend: None
• Mileage Reimbursement: Yes (between patient visits)
• Dress Code: Any solid-color scrubs

Key Responsibilities

• Complete OASIS Start of Care (SOC), Recertification, and Discharge assessments.
• Provide skilled nursing care, including medication management, wound care, and patient education .
• Develop, implement, and evaluate individualized care plans.
• Complete accurate and timely documentation using Homecare Homebase (HCHB) .
• Collaborate with physicians and the interdisciplinary team to ensure quality patient care.

✅ Required Qualifications

• Active Registered Nurse (RN) License
• Home Health experience required
• OASIS experience required
• Homecare Homebase (HCHB) experience required
• Strong clinical assessment, documentation, and communication skills

• Positive attitude and excellent interpersonal skills
• Ability to work independently and effectively manage a patient caseload
