The role involves supporting patients in managing their daily activities and ensuring their comfort and dignity during the end-of-life phase. The aide will work closely with a multidisciplinary team to deliver high-quality hospice care tailored to the needs of each patient. Key Responsibilities: - Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility. - Monitor and report changes in the patient's condition to the hospice team promptly. - Provide emotional support and companionship to patients and their families. - Administer prescribed medications under the supervision of a registered nurse. - Maintain accurate and timely documentation of patient care activities. - Ensure a safe and clean environment for patients by adhering to infection control protocols. - Collaborate with healthcare professionals to develop and implement individualized care plans. - Educate patients and families on hospice philosophy and care practices. - Respect patient confidentiality and uphold ethical standards in all interactions.## QualificationsRequired Education: - High school diploma or equivalent. - Completion of a state-approved Certified Home Health Aide training program. Required Experience: - Minimum of 1 year of experience working as a Certified Home Health Aide, preferably in a hospice setting. - Experience in providing compassionate care to patients with terminal illnesses. Required Skills and Abilities: - Strong understanding of hospice care principles and practices. - Ability to follow a care plan and provide personalized patient care. - Excellent communication skills, both verbal and written, to interact effectively with patients, families, and healthcare team members. - Proficiency in basic patient care techniques, including bathing, dressing, and mobility assistance. - Ability to work independently and manage time effectively in a home setting. - Empathy, patience, and a genuine desire to help improve the quality of life for patients. - Basic computer skills for documentation and communication purposes.
