Healthcare Care Coordinator

Our client is seeking a highly motivated and empathetic individual to join our team as a Care Coordinator. In this critical role, you will be the first point of contact for patients, physicians, and referral sources, providing compassionate support and efficient, first‑call resolution. Location: Remote | Hours: Must be flexible between 7:00 a.m. and 7:00 p.m. CST | Rate: $20.00 per hour. Salary: $19–$20 per hour. Shift: First. Work hours: 7 AM – 7 PM. Education required: High School. Responsibilities Create and complete accurate referrals and applications using our centralized Customer Relationship Management (CRM) platform, staying updated on any policy or procedural changes. Resolve patients' questions and concerns regarding their patient journey status with clarity and empathy. Enter detailed, accurate, and clear information into our company proprietary software while conversing with patients via telephone. Place outbound phone calls for patient follow‑ups or confirmations to ensure timely progression of their accounts. Maintain high‑quality service while providing an empathetic and supportive experience to the patient by confidently controlling the conversation and clearly educating the caller. Interact with patient referral sources, which may include healthcare providers, to efficiently process new applicants. Take ownership of patient accounts from initial contact through final approval or denial, ensuring a seamless experience.
