This role includes a combination of outpatient clinic visits and dialysis rounding, providing comprehensive care to patients with chronic kidney disease and related conditions.Key ResponsibilitiesProvide high-quality, evidence-based care to Urology/Uro-Oncology patients in both clinic settings.Conduct thorough patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians.Perform dialysis rounding, including evaluating patients, managing medications, and coordinating care with dialysis center staff.Document all patient encounters and care plans accurately and efficiently in epic.Communicate clearly with patients, families, clinic staff, and interdisciplinary teams.Support nephrology providers by assisting with patient follow-up, education, and chronic disease management.Participate in quality initiatives, care coordination, and process improvement efforts.Opportunity HighlightsPractice at the top of your license, forming trusted relationships and collaborating with leading physiciansStrong internal referral network and collaborative provider partners across Urology/Uro-Oncology and more than 1,600 Duly providersCompetitive salary, financial bonuses, CME funds, and 401K matchLive and work in the vibrant suburbs of Chicago with excellent schools and convenient access to some of the nation's leading healthcare facilitiesCompensation & BenefitsThe anticipated base compensation range for this position is 100,000-140,000, depending upon the applicant's qualifications and experience. In addition to the base compensation, this position may also be eligible for a sign-on bonus.
